Our multidisciplinary team approach to your care includes physicians, physician assistants, nurse practitioners, registered nurses, registered dieticians, nursing assistants, pharmacists, social workers and case managers, physical and occupational therapists, palliative care practitioners, and your home care team.
Our team works with you to develop a detailed self-care regimen and lifestyle management plan. We strive to build your confidence in following this regimen; we teach you to recognize and manage heart failure symptoms, eat healthy, exercise effectively and know when to call your care providers. Our team delivers this education to you and your caregivers during your hospital stay and beyond.
