The iCare - Thank you for Caring! program provides employees, volunteers, patients and family and friends a way to provide immediate recognition to any employee who contributes to the mission and iCare values of the Hospital.
If you have had an interaction with an employee or observed them going the extra mile or living the iCare values, please take a moment to fill out the form below or to complete the notecard found in your patient packet to acknowledge them for their actions.
If filling out the form below, please be sure to include as much information as possible. Once the form is submitted, we will send a copy of your acknowledgement to the employee as well as enter them into a quarterly drawing for a $50 gift card.
If filling out the notecard found in your patient packet, you may wish to present the card to the person or leave it with the nurses station or receptionist - they will make sure it makes its way to the appropriate person and a copy should be placed in the iCare Thank You Box located in the Telephone Operator Vestibule. So that they are entered into the drawing.
