Cheryl A. Chen, PharmD, BCPS: Dr. Chen received her PharmD at Philadelphia College of Pharmacy. She worked as a staff pharmacist and clinical pharmacist at Mercy Fitzgerald Hospital until 2015. Dr. Chen is a pharmacy systems analyst. She troubleshoots and performs analysis of systems to support pharmacy services.

Ta-Seti Donald, PharmD: Ta-Seti received her Doctor of Pharmacy degree from Temple University School of Pharmacy in 2016. She completed her PGY1 pharmacy residency at Pennsylvania Hospital. Ta-Seti subsequently joined Chester County Hospital as a Unit Based Clinical Pharmacist (UBP) for the intensive care unit/step down unit. Her areas of interests include ambulatory care/transitions of care, antimicrobial stewardship, interdisciplinary collaboration/education, and workflow implementation. Jeanne Duggan-Jones, RPh: Jeanne is the Senior Pharmacy Systems Analyst at Chester County Hospital. She received her Bachelor’s in Biology from the University of Scranton in 1991 followed by a Bachelor’s of Pharmacy Degree from the Philadelphia College of Pharmacy and Science. Jeanne has been at CCH since 1994 hired as a pharmacy student technician, then Clinical Staff Pharmacist, and subsequently taking on the role of the department’s first Pharmacy Systems Analyst in 2004. She was a key contributor in the build and testing of the hospital’s first computerized physician order entry system and has successfully led the pharmacy department thru numerous systems conversions, automation implementations, and beta software validations. She enjoys supporting the pharmacy staff by utilizing technology to optimize pharmacy operations, patient safety, and standardization efforts.

Melissa M. Ilano, PharmD, BCPS: Dr. Ilano received her Doctor of Pharmacy degree from Massachusetts College of Pharmacy and Health Sciences in 2011. Upon graduation, she went on to complete a PGY1 pharmacy practice residency at Geisinger Medical Center in Danville, PA. After completion of her residency, she accepted a clinical pharmacist position at Geisinger Medical Center where she specialized in emergency medicine and adult critical care. She served as the emergency medicine pharmacy team leader and was actively involved in trauma, adult/pediatric code response, and stroke response. In 2017, she joined Penn Medicine Chester County Hospital as a clinical pharmacist working in critical care and internal medicine. Over the years, she has precepted many students and residents and served on various interdisciplinary committees. Her professional areas of interest include critical care, emergency medicine, education, medication safety, and policy implementation. She enjoys spending time with family/friends, cooking, and being outdoors.

My Knapp, PharmD: My Knapp received her Bachelor of Science degree in Pharmacy in 1995 and her PharmD from Temple University's School of Pharmacy in 2000. She began her career as a staff pharmacist at Chester County Hospital in 1997, then moved on to Apria Healthcare Home Infusion from 1999-2008. From 2008-2017, she practiced inpatient hospital pharmacy at Lankenau Medical Center where she was an inpatient coordinator in charge of organizing the pharmacy team to the medical rounds, promoting pharmacy clinical initiatives, initiating and operating the pharmacy's Coumadin clinic, clinical monitoring and managing high-dollar inventory of chemo drugs for both inpatients and outpatients. Now, she is back at Chester County Hospital, where she has served as IV pharmacist and, most recently, as OR pharmacist..

Melissa Lavrich, PharmD: Melissa received her Bachelor of Science degree in Pharmacy from Duquesne University in 1985. She received her PharmD from Shenandoah University in 2014. Melissa has worked for over 20 years in the field of home infusion, began specializing in sterile compounding and was made the IV Production Supervisor at Coram Specialty Infusions in 2010. She joined the staff at Chester County Hospital in the role of Sterile Compounding Pharmacist in March 2020. Melissa’s areas of interest are sterile compounding and USP 797/800 compliance. She is pursuing Board Certification in Sterile Compounding.

Andrew (Andy) Milewski, PharmD, BCPS, BC-ADM: Andy is a graduate of Drexel University (Bachelor of Science in Chemistry, minor in Mathematics) and Temple University School of Pharmacy (Doctor of Pharmacy). He completed a PGY-1 Pharmacy Residency at the Corporal Michael J Crescenz VA Medical Center in Philadelphia, PA. Following residency, he worked as a per diem inpatient pharmacist until joining Penn Medicine in 2019. He currently works as one of four pharmacists in Penn Partners in Care, supporting eight primary care practices with a variety of pharmacy services. He is part of an extensive team of nurses, social workers, and care partners integrated with the staff of family and internal medicine practices. In his daily activities, Andy helps primary care providers manage a variety of chronic diseases, as well as assists with medication-related problems following inpatient admissions. He serves as an APPE preceptor for several pharmacy schools in the area and as a preceptor for the Hospital of the University of Pennsylvania’s Pharmacy PGY-1 transitions of care rotation. Andy has also served on the board of directors for the Delaware Valley Society of Health-System Pharmacists (2020-2022). He holds board certifications in Pharmacotherapy and Advanced Diabetes Management, and his pharmacy interests include precepting of students and residents, chronic disease therapy management, diabetes-related technology, and improving access to medications across the greater population. Outside of work, Andy is an avid runner, hiker, and generally anything outdoors.

Bianca Nguyen, PharmD: Bianca received her PharmD from the Philadelphia College of Pharmacy and went on to complete a PGY-1 at Reading Hospital. She joined the Hospital of the University of Pennsylvania as a clinical pharmacist after residency before starting at Chester County Hospital as a unit-based pharmacist. Her areas of interest include antimicrobial stewardship, internal medicine, and anticoagulation.

Glendon Palomino, PharmD: Glen received his Bachelor of Science in Pharmacy from Temple University School of Pharmacy in 1993. He received his Doctor of Pharmacy from the University of Kansas in 2011. He has worked as a pharmacist in a variety of settings such as hospital, retail community pharmacy, specialty compounding practice, home infusion pharmacy and long-term care consultant pharmacy. Most recently, he worked as a clinical-staff pharmacist in pediatrics at Temple University (old Children Medical Center) for 8 years and as a pharmacist specialist in Neonatal ICU at Lankenau Hospital for 5 years. He joined Chester County Hospital in 2011 as the over-night pharmacist of a 220-bed hospital. In the last 3 years, he as worked as the unit-based pharmacist on two telemetry units and his areas of interests include internal medicine, heart failure and end-stage renal disease.

Reshma Patel, PharmD: Reshma received her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy at the University of the Sciences in Philadelphia in 2011. She started her career as a Clinical Pharmacist at Reading Hospital and Medical Center in West Reading, PA. There she had a deep dive into clinical areas including ICU, Med-Surg, Cardiology/Telemetry, IV room, NICU, ED, Pediatrics and then primarily transitioned to Oncology/Infusion Center. In 2017, she moved onto Brandywine Hospital in Coatesville, PA as initially a Sterile Products Coordinator, then promoted to Pharmacy Manager and subsequently Interim Director/Director of Pharmacy until the hospital’s closure in 2022. She recently joined Chester County Hospital as a Unit Based Clinical Pharmacist (UBP) and is the preceptor for PGY1 Cardiology rotation. Her professional areas of interest include cardiology, sterile products, oncology, education and transitions of care.

Trisha Patel, PharmD, BCPS, BCCCP: Trisha is the Residency Program Director for the PGY1 program. She received her Doctor of Pharmacy degree from University of Georgia School of Pharmacy in 2008. She completed her PGY-1 Pharmacy residency and PGY-2 residency in Critical Care at the University of Alabama Hospital (UAB). She continued to practice as the Medical ICU pharmacist at UAB Hospital for 5 years and then moved to Philadelphia where she worked at the Cancer Treatment Centers of America for a subsequent 5 years as the Infectious Disease / Critical Care Pharmacist in addition to being the Pharmacy Clinical Coordinator. Most recently, she has joined CCH in March of 2020 as the Unit Based Pharmacist (UBP) for the Intensive care unit / step down unit. Her area of interests are critical care, antimicrobial stewardship, education, protocol / guideline development and implementation as well as clinical research. |

Jaclyn M. Seiple, Pharm.D., BCPS: Jaclyn is a Clinical Pharmacist at Chester County Hospital, and is a preceptor for the PGY1 Internal Medicine rotation. She completed undergraduate work and received her doctor of pharmacy degree from the University of the Sciences in Philadelphia in 2007. Jaclyn completed PGY1 residency training at Albert Einstein Medical Center in Philadelphia, PA. She worked as a clinical pharmacist at Wilmington Hospital, Christiana Care in Wilmington, DE after completing her residency in 2008 to 2017. Jaclyn then briefly worked at Perform Rx, Amerihealth Caritas as a clinical pharmacist in the DTM/MTM department in 2018. She joined CCH in November 2018 as Clinical Pharmacist. Jaclyn is an active member of the medication safety committee. She primarily works on the medical/surgical unit as well as the observation unit. Her interests include infectious disease and surgical medical management. |

Connie Shah, PharmD, BCPS, BCGP: Connie obtained her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy at the University of the Sciences in Philadelphia and completed her PGY1 Pharmacy Practice residency at Thomas Jefferson University Hospital. She practiced as a clinical pharmacist in the emergency department at ChristianaCare Christiana Hospital before transitioning to inpatient care as the lead clinical pharmacist on the Acute Care of the Elderly unit and medical/surgical unit at ChristianaCare Wilmington Hospital. She recently joined Chester County Hospital as a clinical pharmacist in the emergency department. Her areas of interest include geriatrics, infectious disease, and transitions of care.

Meryn Sweet, PharmD, BCPS: Meryn obtained her Bachelors of Science Degree from West Chester University in Pharmaceutical Product Development. After working for a year at Wyeth Pharmaceuticals as project manager, she went back to school and obtained her Doctor of Pharmacy Degree from Thomas Jefferson University, followed by a PGY1 Pharmacy Residency at Paoli Hospital. For the past eight years she has worked at Paoli and Chester County Hospital in various areas of pharmacy including OR, med/surg units, main pharmacies, and emergency departments. She recently joined Chester County Hospital full time as a Clinical Pharmacist in the Emergency Department. Her professional areas of interest include: medication safety, transitions of care, and anticoagulation. |

Heather Teufel, Pharm.D., BCCCP: Heather is the Director of Pharmacy at Chester County Hospital. She received her Doctor of Pharmacy degree from Temple University School of Pharmacy. She completed a PGY1 Pharmacy Residency at Allegheny General Hospital and a PGY2 Critical Care Residency at the Virginia Commonwealth University Health System. After residency training, she worked as the Medical ICU Clinical Pharmacy Specialist at Penn State Hershey Medical Center, then as a Surgical ICU Clinical Pharmacy Specialist at the Hospital of the University of Pennsylvania. She joined the team at Chester County Hospital in 2015 as one of the first Emergency Medicine Clinical Pharmacists, before transitioning to the Associate Director of Pharmacy position and ultimately to her current role. Her clinical interests include sepsis, acute ischemic stroke, antimicrobial stewardship, opioid use disorder and opioid stewardship. |

Hanan Urick, PharmD, BCOP: Hanan received her Bachelor of Science degree in Pharmacy from Philadelphia College of Pharmacy & Science in 1994. She received her PharmD from Shenandoah University in 2001. She earned her Board Certification in Oncology(BCOP) in 2017. She began her career as a staff pharmacist at Rite Aid in Cliffside Park, NJ until 1996 when she joined Geisinger Medical Center in Danville, PA as a staff inpatient pharmacist from 1996 to 2005. She practiced in the areas of Thoracic Medicine/Infectious Disease, Intravenous Admixture Services and was the night shift pharmacist for the 500 bed trauma center from 2002-2005. She joined the staff at Chester County Hospital in 2007 and practiced in the inpatient area and the employee pharmacy. She started practicing in the outpatient Oncology Infusion Service in 2010 and currently works at the Abramson Cancer Center in West Chester as well as covering the inpatient pharmacy at the hospital on weekends and holidays. She most recently completed certificate course in USP 797 and USP 800.

Eric Urnoski, PharmD: Eric obtained his Doctor of Pharmacy degree from Thomas Jefferson University and completed a PGY-1 Residency at Hackensack University Medical Center. He then worked as a Clinical Pharmacy Specialist in Medical Intensive Care at Crozer-Chester Medical Center for nearly 7 years prior to joining Penn Medicine. He serves as a preceptor for the required Infectious Disease rotation and supervises residents responding to code blue alerts. His professional interests include: multidrug resistance, substance use disorder, and toxicology.

Chelsea Zavilla, PharmD, BCPS, MPS: Chelsea graduated from the University of Pittsburgh School of Pharmacy and then completed her two-year Health System Pharmacy Administration residency at the University of Virginia Health System. During her residency, Chelsea earned her Masters of Pharmaceutical Sciences degree and a Teaching and Learning certificate from Virginia Commonwealth University. After residency, she served as the Clinical Pharmacy Manager and PGY1 Residency Program Director at Bon Secours Memorial Regional Medical Center in the Richmond, VA, area. In May 2022, Chelsea joined the Chester County Hospital team as the Associate Director of Pharmacy and currently serves as a preceptor for the Pharmacy Administration residency rotation.
