The Health System works cooperatively with the Chester County Health Department and several other local health organizations to conduct a community needs assessment. The process is called RoadMAPP to Health: Community Health Assessment for Chester County. Involved in this effort are leaders from all sectors of the area to represent health, housing, access to food, social services, faith centers, schools, various foundations, and many others. This group is conducting a comprehensive survey to identify and then prioritize needs and mobilize actions to address them. This is an ongoing process.

PARTNERSHIPS
With the goal of providing the most efficient and coordinated network of care possible for its patients, meeting their needs has led the Hospital to reach out to its surrounding community and develop many valuable and lasting partnerships and collaborations. These partnerships create the opportunity to bring meaningful health education to a wider audience and to collaborate on initiatives.
