A vaccine card is validation that you received the COVID-19 Vaccine. Ask for a vaccine card at your appointment.
Depending on which vaccine you receive, you may require a 2nd dose. The following valuable medical information is on the CDC approved card:
- Which vaccine you received
- The date you received it
- The location you received it
What Should You Do With Your COVID-19 Vaccine Card To Keep It Safe?
- Make a few copies
- Take a few pictures front/back of card
- Keep it in a safe place
- Do not laminate the card (in case of updates)
- Do not take pictures and post on social media (identify theft)
What Should You Do If You Lost Your COVID-19 Vaccine Card?
- State of PA (except City of Philadelphia): Email the PA-SIIS at ra-dhpasiis@pa.gov, and you will receive a form to fill out and send back to get your vaccination records.

- State of DE: Send an email to Vaccine@delaware.gov (subject line: Lost Vaccine Card) and include: Full Name / Date of Birth / Date & Location you received your vaccine
