A vaccine card is validation that you received the COVID-19 Vaccine. Ask for a vaccine card at your appointment. 

Depending on which vaccine you receive, you may require a 2nd dose. The following valuable medical information is on the CDC approved card: 

  • Which vaccine you received
  • The date you received it
  • The location you received it

What Should You Do With Your COVID-19 Vaccine Card To Keep It Safe?

  • Make a few copies
  • Take a few pictures front/back of card
  • Keep it in a safe place
  • Do not laminate the card (in case of updates)
  • Do not take pictures and post on social media (identify theft)

What Should You Do If You Lost Your COVID-19 Vaccine Card?

  • State of PA (except City of Philadelphia): Email the PA-SIIS at ra-dhpasiis@pa.gov, and you will receive a form to fill out and send back to get your vaccination records.
  • City of Philadelphia: covid@phila.gov
  • State of NJ: Website: njiis.nj.gov > NJIIS Click "Request Immunization Record"
  • State of DE: Send an email to Vaccine@delaware.gov (subject line: Lost Vaccine Card) and include: Full Name / Date of Birth / Date & Location you received your vaccine

  • If you enrolled in v-safe or VaxText, you can access your vaccine information using those tools.
  • MyChart by myPennMedicine: Access if you used to schedule your vaccine (Health Summary – Immunizations)
