The Tribute Program & Health Care Hero offer thoughtful ways to make a gift to remember a loved one or friend, or to recognize your doctor, nurse or other caregiver.
Click Here to view the names of the individuals who were honored or memorialized in Fiscal Year 2020. In addition, Chester County Hospital Health Care Heroes will receive special recognition from the Foundation.
With your tribute gifts, please include the full name of the person(s) being honored or memorialized and the contact information for any person or family member that should be notified of your thoughtful donation. We will send a letter to the individual being honored or to the family of the individual who has passed expressing your thoughtfulness. For Health Care Hero tribute gifts, please include the name of Chester County Hospital doctor, nurse, or other team member and the Hero’s specialty and location (practice/office, department, area, floor, etc.). Please also write a note of thanks for your Health Care Hero and tell us why they mean so much to you.
