For more than 100 years, Neighborhood Health Agencies and Neighborhood Visiting Nurses Association have been trusted and  valued members of the Chester County Community. They have consistently provided home health and outreach services that have enhanced the health and well-being of patients, families, and our community.

The evolution of Neighborhood Health has grown over the years. In 1995, the organization became a subsidiary of Chester County Hospital and Health System. When Chester County Hospital joined Penn Medicine in 2013, Neighborhood Health became part of the Penn Medicine health system as well. Since then, Neighborhood Health Agencies and Neighborhood Visiting Nurses Association have been a part of Penn Home Care and Hospice Services, Penn Medicine’s entity providing home-based care.

On March 10, 2019, Penn Home Care and Hospice Services was renamed Penn Medicine at Home. Correspondingly, the home health services previously provided by Neighborhood Health Agencies and Neighborhood Visiting Nurse Association began being provided by Penn Medicine at Home through Penn Medicine Home Health and Penn Medicine Hospice.

Care continues to be provided by the same staff, with the same commitment to best-in-class, compassionate care. Penn Medicine at Home’s commitment to the community remains strong, with patients and their families at the center of all they do!
